Care delivered as part of Stony Brook Medicine is governed by that organization’s privacy practices and its Notice of Privacy Practices, provided to patients at the point of care.

We collect only what you choose to send us through the appointment request form or the Front Desk assistant: your name, phone number, optional email, preferred office, and a brief, optional description of your reason for the visit. We collect this for the sole purpose of contacting you about scheduling.
Please do not submit sensitive medical details, test results, or other protected health information through this Site. These tools are for scheduling, not for secure health communication, and they are not intended for emergencies.

We do not sell your information, and we do not use it for advertising.
How information is processed
Form submissions may be transmitted and temporarily stored by a third-party form-processing service acting on our behalf so the office can receive your message. That provider processes the data only to deliver it to us.

The Site is designed to run without advertising or cross-site tracking cookies. If basic, privacy-respecting analytics are added in the future to understand general traffic, this policy will be updated to describe them.
Data retention & security
We keep appointment-request information only as long as needed to respond and to maintain ordinary business records, and we take reasonable measures to protect it. No method of transmission over the internet is completely secure.

The Site is intended for adults and is not directed to children under 13, and we do not knowingly collect their information.
